Why Record Medical Information?

Accurate medical information is essential for child safety and may be required for certain funding claims (e.g., Disability Access Fund).

Information to Record

• Allergies - Food allergies, environmental allergies, medication allergies
• Medical Conditions - Asthma, diabetes, epilepsy, etc.
• Medications - Regular medications the child takes
• Dietary Requirements - Vegetarian, halal, gluten-free, etc.
• Emergency Contacts - Who to contact in an emergency

SEND Information

For children with Special Educational Needs and Disabilities (SEND), record:

• Whether the child has SEND
• Details of their needs
• Whether they have an EHCP (Education, Health and Care Plan)
• Support worker details if applicable
