About the GOV PDF Form
The system generates official DfE-styled parental declaration forms that match the government's standard format. These forms are used for funding claims and can be printed or saved as PDF.
Form Sections

*Step 6 shows custom terms if configured; otherwise declaration is Step 6.
Ethnicity Code Table
The form includes a visual ethnicity code table displaying all 21 DfE ethnicity codes. The child's selected ethnicity is highlighted in green. This helps ensure ethnicity data matches local authority reporting requirements.
Document Recording
The 'Document recorded by' field automatically shows all nursery managers assigned to the nursery. This appears as: Jane Smith - Nursery Manager, John Doe - Deputy Manager
Managers are identified by the 'Is Manager' flag in their user profile's nursery assignments.
Additional Charges Disclosure
A notice appears before the extras section explaining:
- Government funding covers free hours only
- Additional charges may apply for optional extras
- Costs should be published on provider websites
Company Logo
Your company logo appears in the top-right corner of the PDF. Upload your logo in System Settings > Company Branding.
Tip: To add custom terms (Step 6), configure them in the nursery's settings page.
